Items with an asterisk are required.

May we suggest that you print out the form and take time to thoughtfully complete your information and then type it on the form once you have all the data. As with any web form, it may "time-out" if open for an extended period while you gather thoughts (and information) and you may find yourself in the unfortunate position of having to retype everything.

Please be thorough. Instead of "Sports Club" plese list the name of the club and which years/seasons you participated. State specifically when you served in a position. All this will help the committee process.

Awards will be presented on Tuesday, April 29, in the Student Center Ballroom at A Celebration of University Leadership. Refreshments and entertainment will begin at 3:00 p.m.; the awards program starts at 3:30 p.m.
