Online Requirement Fulfillment Form
To make it easier to inform your CLP advisor of any requirements you have completed, simply fill out this online form.   Make sure you provide all relevant information on the type of requirement you have completed, the date you completed it, and a brief description of what you did.  Fields in red are required.  Please complete a different form for each requirement.

Please give a brief description (at least a paragraph) of the activity in which you participated.  For example, if you visited a museum, you can describe the nature of the exhibit you visited, your favorite part of the exhibit, or something you learned.  For a lecture or workshop, describe both the nature and content of he event.  If you visited Career Services, please describe what you did there (revise a resume, engage in career counseling, etc.)
